From its development and first use over a century ago, radiography — the use of x-rays to view internal images in the body — has revolutionized how dentists diagnose and treat patients. Now, a new technology known as Cone Beam Computing Tomography (CBCT) promises to take us “light years” beyond even today's most modern conventional x-ray devices.

X-rays expose images on special film after passing through a mass, like the human body. Because they pass more easily through soft tissues than through hard structures like teeth or bone, the softer tissues will appear darker. This property can reveal even subtle distinctions in density such as might be the case with a fracture or a tooth cavity.

Standard radiography, though, has its limitations. It takes extensive training and experience for a dentist to interpret exactly what they're seeing in an x-ray. Their two-dimensionality (like a photograph) limits the amount of information we can derive from the physical structures being examined. And due to radiation exposure to patients, we must limit the amount of their use for each individual patient.

CBCT improves on those limitations. The device projects a cone-shaped beam of x-rays as it rotates around a patient's head. During this rotation it records hundreds of images that a computer can later digitally format in a variety of ways. The result: instead of a two-dimensional flat view, we can now three-dimensionally view the mouth from a variety of different angles and in greater detail. Best of all, one scan can provide enough imagery data to view in detail the entire skull or a jaw, or something as minute as a single root canal within a tooth.

CBCT is already improving the accuracy of diagnostics and treatment in a variety of dental specialties, including orthodontics, implantation and oral surgery. And properly set, the radiation exposure is no more or less than a full-mouth series of x-rays, and up to ten times less than CT scanning.

Advances like CBCT increase the range and accuracy of diagnostics and improve treatment for a variety of conditions. As they grow in use, the result will be more successful dental outcomes for you and your family.

As the youngest person ever to host Entertainment Tonight, Maria Menounos, an independent filmmaker, actress, and co-host of daily entertainment news program Extra, has made a huge splash in the world of entertainment journalism. However, she is also an avid ambassador for the American Diabetes Association, a cause that is very dear to her heart because her father is a diabetic.

Her father's illness taught Menounos and her family about the importance of maintaining good general and dental health. This included a diet packed with fruits and vegetables, many of which they raised themselves. According to Menounos, they also ate little-to-no junk food. These habits still help keep the busy celebrity journalist fit and smiling with beautiful, healthy teeth.

Speaking of her smile, Menounos openly discusses her oral health in her interview with Dear Doctor magazine. She has had no major dental enhancements — not even braces — but does occasionally brighten her smile with tooth whitening. She also feels that her teeth are healthy due to the sealants she had as a child.

While periodontal disease can take on a variety of forms, most are caused by a thin layer of bacterial plaque called biofilm. This layer of plaque will form every 8-12 hours and sticks like glue to your teeth near the gum line. With time, tartar formation occurs at and below the gum line.

If left unchecked, biofilm can give rise to a very unhealthy progression. It first triggers an infection that leads to painful inflammation, progressive bone loss and the gum tissue losing attachment with a tooth. Void spaces (or pockets) form where the gum and bone tissue once adhered; infectious plaque and tartar moves into these pockets and advances deeper to the root. Overcome by disease, the tooth is in danger of being lost.

It's imperative then to remove as much of this entrenched plaque and tartar as possible. Renewed oral hygiene is not enough — removing plaque and tartar from the root surfaces requires a treatment known as root planing.

Root planing is a meticulous, labor-intensive process. We first clear away larger portions of plaque around the teeth and gums with hand instruments or an ultrasonic device and then flush out the pockets with water. After administering a local anesthetic for pain, we would then turn to a number of small hand instruments known as curettes to probe and scrape away as much remaining plaque below the gum line as we can get to.

Root planing requires experience and a good sense of touch to work in areas that can't be clearly seen. Observing the gum line, though, can give us a good indication of progress as these tissues will actually change color once the biofilm and tartar deposits have been removed.

Being so deeply entrenched, not all the deposits might be removed during one session. However, as plaque and tartar are removed, the gum tissues will begin to heal and become less inflamed. This will make it easier to remove plaque in subsequent sessions.

Root planing takes time, but the effort is well worth it. In the short term you'll notice less inflammation and pain around your teeth and gums. In the long-term, it just may save your teeth.

Mike Tyson's gap-toothed smile is part of athlete-turned-celebrity's signature look. During his two-decade career as a professional boxer, the former heavyweight champion has been known for both giving — and occasionally receiving — knockout punches. But the story of how he lost one set of front teeth is a bit more unusual.

In a recent interview with the Las Vegas Review Journal, Tyson's wife Kiki stated that one of the champ's major dental dilemmas didn't come from blows inside the ring. In fact, she said, Tyson lost the teeth after being head-butted by his pet tiger, Kenya.

It's too bad Tyson wasn't wearing a mouthguard before he decided to play with kitty.

Fight fans know that boxers always put in a mouthguard before they enter the ring. But the pugilistic pursuit is just one among the two-dozen-odd sports for which the American Dental Association recommends the use of custom mouthguards. Others include baseball, skateboarding, surfing and bicycling. (Maybe horsing around with tigers should be added to the list!)

Why is it so important for participants in athletic activities to use this piece of protective gear? According to the U.S. Centers for Disease Control, sports-related dental injuries account for over 600,000 emergency-room visits each year. Not wearing a mouthguard makes an athlete 60 times more likely to sustain harm to the teeth, according to the American Dental Association. So there's really no contest.

Orthodontic treatment (commonly known as braces) can be a lengthy process to re-align your teeth to a more functional and aesthetic position. Once the orthodontic devices are removed, however, the treatment isn't finished. Wearing a retainer is the final step to ensuring that the re-alignment doesn't eventually fail. It's designed to do just what its name implies — to “retain” the teeth's new position and prevent a relapse to the old.

This can happen because of the way teeth fit into the jaw bone. The teeth are joined to the bone by the periodontal ligament, which works somewhat like a hammock: the ligament's fibers act like threads that fit into the tooth on one side and into the bone on the other, and hold the teeth in place.

As living tissue, the ligament's cell structure is dynamic and can adapt to the gentle pressure applied by an orthodontic device. However, once this pressure subsides after the device is removed “muscle memory” can cause the ligament to resist the new position and pull the teeth back to their original setting. The retainer helps hold the teeth in the new position while the bone and ligament continue to mature and stabilize around the teeth.

There are two basic types of retainers; the one recommended for you will depend on your age and the extent of your orthodontic treatment. One type is a removable device that is typically worn around the clock initially, but may eventually only need to be worn at night or for even a lesser interval of time. The other type is attached permanently behind the teeth and can only be removed by an orthodontist. Permanent retainers have the benefit of not being as visible as the removable type, and there's no bother with putting them in and taking them out.

You may consider wearing a retainer a nuisance especially after months of orthodontic treatment. But consider it the last lap in a long race — only by finishing can you achieve that winning smile.
